Rolling around

A 25 year old man presents with right sided severe abdominal pain radiating from the loin to the groin. He keeps on writhing in pain and is unable to lie still. He denies frequency, dysuria or hematuria and is afebrile. This is his first such episode. He is otherwise asymptomatic and his medical history is unremarkable.
