Out of comfort

A 45 year old man presents with progressively worsening dyspnea and chest discomfort for 2 weeks, which became significantly worse over the last 4 days. He also gives a history of feverishness and malaise for the preceding 3 months. He is otherwise asymptomatic and his medical history is unremarkable.
